When is the ball out?

From USA Rugby High Performance Manager Richard Every

If the ball is on the ground, and a player in the scrum half position is holding the ball, with one or two hands, the ball is not out, unless the ball is clear of the ruck from a bird’s eye view. If the player picks up the ball it is deemed to be out. This does not allow players who are being driven backwards in a ruck to use their hands to retain/secure possession.
